It is estimated that in up to 30% of couples with fertility problems, there is a male factor contributing. I have a special interest in male fertility. I believe that it is very important for the man to be properly evaluated. A full enquiry into lifestyle factors, past history and general health is undertaken. A physical examination may detect obvious and potentially treatable conditions. Often, special tests are needed to properly evaluate the man.

Improving male fertility

When we embark on improving male fertility, we look at your general health and lifestyle, including your diet and your level of exercise. These are some of the aspects we may discuss together:

  • Treating underlying conditions, such as high blood pressure, and diabetes
  • Avoiding certain drugs and medications which may impair sperm production
  • Improving low hormone levels
  • Avoidance of smoking
  • Reducing alcohol intake
  • Wearing loose-fitting underwear and avoiding hot baths and saunas in order to reduce testicular temperature
  • Minimising exposure to environmental chemicals
  • A diet rich in nutrients including antioxidants. Including more fruit and vegetables in the diet
  • Regular exercise
  • Managing stress
  • Advice about frequency and timing of sex

Sometimes I advise surgical correction of conditions. Examples include treatment of undescended testes and varicocele.

Fertility tests for men in order to determine if there is a male factor causing infertility

A physical examination may yield unsuspected problems such as a varicocele or undescended testis which may need treatment. Absent vas deferens from birth is also relatively common. Fertility testing in the Clinic also involves highly specialised evaluation of the sperm. This includes:

  • Ejaculate volume. There are some important conditions which affect volume
  • Numbers of sperm
  • Movement of sperm (and importantly the rapid, forward motion of the sperm)
  • The shapes of the sperm
  • Sperm Chromatin Structure Assay test. This looks at the percentage of sperm showing fragmented DNA and helps to predict which men are less likely to achieve a healthy pregnancy either naturally or with IVF
  • Testing for the presence of anti-sperm antibodies
  • Blood tests to check for hormonal derangement and chromosomal disorders; Sometimes specialised genetic tests are undertaken

Micro TESE

Micro TESE is a highly specialised procedure and is undertaken to extract sperm from the testes. We use it in cases where there is no sperm reaching the ejaculate. This is caused by damage to the seminiferous (sperm-making) tubules in the testes.

TESA

In cases where there is no damage to sperm production in the testes, but the azoospermia is caused by mechanical interruption to the flow of sperm, then it is a much simpler procedure to extract the sperm. This is called the needle TESA procedure.

Vasectomy reversal

A vasectomy reversal procedure is undertaken in order to undo the vasectomy sterilisation. In this technique, the vas deferens are re-joined to allow natural conception. Typical rates for sperm presence in the ejaculate following reversal surgery are around 90%.
